What Causes Repetitive Stress Injuries in the Workplace?

Some workplace injuries happen in a single identifiable event. Repetitive stress injuries develop differently. Repeated movements, sustained force, vibration, awkward positions, and other physical demands can gradually affect muscles, tendons, nerves, ligaments, joints, and related tissues. The National Institute for Occupational Safety and Health (NIOSH) groups many of these conditions under work-related musculoskeletal disorders, or WMSDs.

Repetition is only one part of the problem. NIOSH identifies force, posture, frequency, duration, vibration, and other workplace conditions as factors that can contribute to musculoskeletal disorders. Several risk factors can also occur during the same task, increasing the physical demands placed on a particular part of the body.

Repeating the Same Movement for Long Periods

Frequent repetition can repeatedly load the same muscles, tendons, or joints without enough variation in movement. NIOSH identifies repetitive work as a risk factor associated with conditions affecting areas such as the neck, shoulders, elbows, hands, and wrists. Repetitive motion is particularly concerning when it occurs together with forceful exertion or an awkward position.

Title 8, Section 5110 applies when more than one employee has suffered an objectively diagnosed musculoskeletal repetitive motion injury within the prescribed period while performing identical work activity, and the injuries were predominantly caused by the repetitive job, process, or operation.

When the regulation applies, the employer must establish and implement a program designed to minimize repetitive motion injuries. Section 5110 identifies three areas the program must address: worksite evaluation, control of exposures that caused the RMIs, and employee training. The rule demonstrates that California treats repetitive injury prevention as an ergonomic problem involving the design and management of work, not simply the physical characteristics of an individual employee.

Repetitive Stress Injuries Usually Require Looking at the Entire Task

Repetitive stress injuries don’t arise from repetition alone. Factors such as the force used, awkward positions, task speed, exposure duration, vibration, and how tools fit the task all matter. NIOSH’s ergonomic framework considers these factors together, not just one movement.

This approach helps explain why workers doing simple tasks can have different musculoskeletal issues.
